The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. Our Care Team Specialist (CTS) are part of a team that provides compassionate care to the frailest elderly who suffer from chronic and acute illnesses or injuries living in their homes.  Our CTS is responsible for answering incoming and making outgoing calls, helping coordinate care with the rest of the medical team, scheduling patient appointments with our team, specialist appointments and coordinating diagnostic tests, being a community resource, working to engage patients that currently are not engaged, and contributing complete and accurate data in patient's records. Through this approach and the incredible care made possible by WellBe’s clinical teams, we are improving quality of life, reducing healthcare costs, and addressing unique challenges for truly underserved patients.   GENERAL SUMMARY Reporting to the Chief Growth Officer, the Director of Account Management oversees client support and serves as the primary liaison to WellBe’s key partners including national and regional health plans, risk-bearing medical groups and other healthcare organizations. This role will work directly with WellBe leadership, clinical and operational teams to ensure partner success, develop exceptional client relationships, and meet all financial, operational, and quality objectives. The Director of Account Management is responsible for developing the strategy and service roadmap for growth in partnership with the company’s Chief Growth Officer, Founder/CEO, and Chief Financial Officer.
