The ideal candidate will have demonstrated proficiency of healthcare data sets, including medical claims, HCC, HEDIS, and be detailed oriented.  This role reports to the SVP of Data and Clinical Solutions and works closely with operational and clinical leaders. The Reporting Consultant Sr will have ownership and is responsible for developing all companywide dashboards, reports, metrics and key performance indicators assets while also providing in-depth analysis and recommendations for improving/enhancing market performance. Documents, evaluates, and improves design of reports and processes to ensure that analytical tools evolve as the business changes. Develop trend analyses and other quantitative reports to determine business trends. Is an active participant in process improvement and problem resolution issues. Identify key insights and communicate to various stakeholders through the WellBe organization.

Daily duties include managing staff, ensuring compliance with regulations, and maintaining the overall efficiency and quality of the practice. This role will be involved in strategic planning, marketing, and implementing policies and procedures to improve patient care and organizational effectiveness. Additionally, practice managers often act as a liaison between healthcare providers, staff, and patients, playing a vital role in the smooth running of the practice.

They will be required to be hands on and develop metrics, algorithms, and workflows related to forecasting RAF/value, supporting prospective point of care tools, while supporting and developing the analytics team. This individual will interface with senior leaders and will need to have excellent communication and presentation skills. This position will also be required to foster growth and support for analysts on the team, even though it is an individual contributor role.

This position requires travel.

You will have strong ownership and accountability over creating new data transfer channels and managing and maintaining the successful daily operations of receiving and delivering files supporting new and existing customers and partners. As a Data Integration Engineer you will be a member of our highly technical production support team, who develops, pro-actively monitors, performs wellness checks and observes the status of File Transfer in the Production and Development environments. You will be focused on the daily goal of meeting established business Service Level Agreements (SLAs) and/or Service Level Objectives (SLOs) for WellBe’s internal/external customers, through the utilization of the monitoring and triage tools provided, with emphasis on problem resolution, root cause investigation, providing level one and two problem detection, determination and resolution function, vendor engagement, maintenance of event/outage logs and assistance with hardware and software patch/product updates. You will collaborate with cross-functional teams, including data scientists and business analysts, to develop data pipelines and implement analytical solutions. Additionally, you will monitor and manage support ticket queues, address end user issues and ensure solutions meet or exceed WellBe data security and privacy controls.

In the role, you will be responsible for designing, implementing, and maintaining a robust healthcare data warehouse that serves as the foundation for actuarial services, data analytics, reporting, and insights. As a key member of our team, you will play a critical role in shaping our data architecture and ensuring that it supports our healthcare data needs effectively. The role is responsible for identifying WellBe’s internal and external data sources, collaborating with department heads to determine their data storage and organizational needs and using the information to create and maintain data infrastructure for company employees. The ideal candidate is an experienced data engineer and data wrangler who enjoys optimizing data systems and building them from the ground up. They must be self-directed and comfortable supporting the data needs of multiple teams, systems, and products. Experience in analytics and statistics is a major bonus.   Experience with claims data specification is a must.

The Engagement Specialist joins the WellBe team’s efforts in acquiring new patients and helping our patients to lead a more meaningful life. This unique role provides the opportunity to directly contribute to WellBe’s growth as well as improve our ability to help people receive WellBe’s life-changing care.   We need you to be an expert at communication and building relationships in order to engage patients and schedule in-home visits from WellBe. A normal ‘day in the life’ consists of conducting outbound calls to prospective patients to schedule in-home comprehensive wellness visits and health risk assessments. The Engagement Specialist connects with potential new patients, provides answers to their questions, and supports our in-home care team to engage patients in their health. The Engagement Specialist is truly the first point of contact for a patient and will be responsible for introducing the patient to WellBe and all the great benefits they qualify for. The Engagement Specialist oversees the monitoring and measuring of call outcomes and works with the engagement team to maximize our ability to enroll new patients.   Engagement is the starting point for WellBe to support people who are in need, and this role is a critical part of our commitment to our patients. Our Patient Care Coordinator (PCC) are part of a team that provides compassionate care to the frailest elderly who suffer from chronic and acute illnesses or injuries living in their homes.  Our PCC is responsible for answering incoming and making outgoing calls, helping coordinate care with the rest of the medical team, scheduling patient appointments with our team, specialist appointments and coordinating diagnostic tests, being a community resource, working to engage patients that currently are not engaged, and contributing complete and accurate data in patient's records. The successful candidate will love working with seniors and a collaborative interdisciplinary team. They deliver primary, urgent and acute medical care to a wide variety of patients. The Nurse Practitioner will be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments to patients as required. The successful candidate will work as a team with our physicians, social workers, paramedics, and care team coordinators to assist in delivering concierge care to every patient. WELLBE INTRODUCTION The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.   GENERAL SUMMARY We are looking for an individual with deep transformational change experience in community-based care who is looking for an exciting challenge to join WellBe’s Performance Management Team to help transform the organization through data-driven process improvement.  This is a high-visibility, “in-the-trenches” role and will include daily interaction with our Performance Management Team, field operations, IT and analytics teams. You will be expected to leverage your transformational change expertise to help identify opportunities to minimize defects and improve process outcomes, while demonstrating an ability to produce executive level deliverables and solid judgement when solving problems.   Some of your work products will include financial analyses, process performance reports, process flows, group meeting facilitation, and interdepartmental relationship development. The ideal candidate will be a high-performing hands-on individual who has proven delivery with a strong understanding of operations and technology. You will have excellent communication skills, attention to detail and the ability to form the big picture. This candidate will also be responsible for care coordination needs and supporting WellBe clinicians with operational and clinical tasks.
