They deliver primary, urgent and acute medical care to a wide variety of patients. The Nurse Practitioner will be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments to patients as required. This leadership role supports the company’s overall business strategy and the daily implementation for all field education activities and quality patient care related functions and performance of the assigned geography and direct reports. Responsibilities include providing a consistent, clear onboarding process for all social workers with their specific regional market community teams. The Clinical SW Supervisor will work to ensure market social workers are educated, motivated, and aligned with the WellBe model of care. The Clnical Supervisor will ensure all social workers in market territories are properly engaged and maintain appropriate communication with those necessary.   Our social workers are part of a team that provides compassionate care to the frailest elderly who have chronic and acute illnesses or injuries living in their homes. Our LCSW delivers primary, urgent and acute care and counseling to a wide variety of patients with social and behavioral problems. The LCSW will be accountable for caring for patients, connecting patients and their families to support services, maintaining accurate and current patient records and scheduling and administering initial, urgent, and follow-up appointments to patients as required. The successful candidate will work as a team with our physicians, advanced practice clinicians, and care team coordinator will assist in delivering quality care to every patient. Accurately noting all patient information according to confidentiality standards and HIPPA requirements.  Routes calls to the appropriate departments as needed.

The successful candidate will employ critical thinking and decision making, be comfortable with technology, have flexibility, and love working with patients and a collaborative interdisciplinary team. Additionally, will be responsible on how to use along with their team; the daily operating systems/tools, familiar and adhere to company policies and procedures, address questions/complaints (team and members), and drive change management. Also assist in hiring new employees while monitoring their team's success on an ongoing basis. WELLBE INTRODUCTION The WellBe care model is a Physician Led Advanced Practice geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.   GENERAL SUMMARY This role will advance WellBe’s understanding of our patient's healthcare utilization and how we can reduce unnecessary healthcare costs while achieving better patient outcomes, and patient experience. The ideal candidate will have a solid knowledge/understanding of healthcare data sets, including medical claims, HCC, HEDIS, and be detail oriented.  This role reports to the Chief Executive Office and works closely with operational, IT and clinical leaders. Responsibilities include collaborating with finance, operational, IT and clinical leaders to design analyses to answer key questions about our care model, complete in-depth analyses leveraging medical claims, EMR inputs, patient attributions, and healthcare operations data sets.   Identify key insights and communicate to various stakeholders through the WellBe organization.
