They deliver primary, urgent and acute medical care to a wide variety of patients. The Nurse Practitioner will be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments to patients as required. The successful candidate will work as a team with our physicians, social workers, paramedics, and care team coordinators to assist in delivering concierge care to every patient. More specifically, the analyst will be responsible for analyzing WellBe’s patient population and generating the optimal daily outreach list to ensure we are contacting the right patient, at the right time. This position will analyze, develop, and design call list strategies through data analytics methodologies and best practices to ensure ongoing performance improvement and alignment of member outreach scheduling (call lists) to forecasted, market and business needs.  This position will leverage system and data knowledge to create and maintain outreach specific reporting in various data visualization tools.  This position will serve as the Subject Matter Expert between our Markets and Outreach Center to ensure calling algorithms align to the optimization of neighborhood design, routing and outreach dialing algorithms. This role will be critical in matching internal supply with external patient demand.

They deliver primary, urgent and acute medical care to a wide variety of patients. The Nurse Practitioner will be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments to patients as required. The successful candidate will work as a team with our physicians, social workers, paramedics, and care team coordinators to assist in delivering concierge care to every patient. Our LISWs are part of a team that provides compassionate care to the frailest elderly who have chronic and acute illnesses or injuries living in their homes. Our LISW delivers primary, urgent and acute care and counseling to a wide variety of patients with social and behavioral problems. The LISW will be accountable for caring for patients, connecting patients and their families to support services, maintaining accurate and current patient records and scheduling and administering initial, urgent, and follow-up appointments to patients as required. The successful candidate will work as a team with our physicians, advanced practice clinicians, and care team coordinator will assist in delivering quality care to every patient. The Engagement Specialist joins the WellBe team’s efforts in acquiring new patients and helping our patients to lead a more meaningful life. This unique role provides the opportunity to directly contribute to WellBe’s growth as well as improve our ability to help people receive WellBe’s life-changing care.   We need you to be an expert at communication and building relationships in order to engage patients and schedule in-home visits from WellBe. A normal ‘day in the life’ consists of conducting outbound calls to prospective patients to schedule in-home comprehensive wellness visits and health risk assessments. The Engagement Specialist connects with potential new patients, provides answers to their questions, and supports our in-home care team to engage patients in their health. The Engagement Specialist is truly the first point of contact for a patient and will be responsible for introducing the patient to WellBe and all the great benefits they qualify for. The Engagement Specialist oversees the monitoring and measuring of call outcomes and works with the engagement team to maximize our ability to enroll new patients.   Engagement is the starting point for WellBe to support people who are in need, and this role is a critical part of our commitment to our patients. WellBe is a Physician Led, Advanced Practice clinician driven, geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically undeserved and very challenged with access to care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. WellBe's care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.
