The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.   GENERAL SUMMARY At WellBe Senior Medical our Data Scientist applies advanced data analysis tools and methodologies to deliver insights and actionable recommendations for the business, as well as individual units. Utilizes complex statistical modeling to forecast future outcomes and in various business scenarios. Simplifies results and communicates findings to business leaders.

GENERAL SUMMARY The Help Desk Junior Technician provides executive level technical support to internal customers experiencing computer hardware and software issues. They respond to queries mostly over the phone and occasionally in person, assess and diagnose computer hardware and software problems and implement effective solutions. This is a ground floor opportunity in an exciting and fast paced start-up. Under the direction of the Dir. Of End User Services, in this position, you will use your current knowledge of the user and technical support industry including best practices, tools, and trends to offer support and technical assistance to internal associates and external customers. To ensure success you should have advanced knowledge of computer software and hardware systems, excellent diagnostic, high-level communication skills, and patience. A strong foundational understand of IT systems, infrastructure, networking, SaaS applications, and end-user operations is required for this position. You must have a passion for supporting and enabling end-users with their technology daily and delivering white glove user centric support across a distributed technically diverse user community. The Engagement Specialist joins the WellBe team’s efforts in acquiring new patients and helping our patients to lead a more meaningful life. This unique role provides the opportunity to directly contribute to WellBe’s growth as well as improve our ability to help people receive WellBe’s life-changing care.   We need you to be an expert at communication and building relationships in order to engage patients and schedule in-home visits from WellBe. A normal ‘day in the life’ consists of conducting outbound calls to prospective patients to schedule in-home comprehensive wellness visits and health risk assessments. The Engagement Specialist connects with potential new patients, provides answers to their questions, and supports our in-home care team to engage patients in their health. The Engagement Specialist is truly the first point of contact for a patient and will be responsible for introducing the patient to WellBe and all the great benefits they qualify for. The Engagement Specialist oversees the monitoring and measuring of call outcomes and works with the engagement team to maximize our ability to enroll new patients.   Engagement is the starting point for WellBe to support people who are in need, and this role is a critical part of our commitment to our patients. WellBe's physician-led geriatric care teams provide “concierge” level geriatric medical care and social support in the home as well as delivering and coordinating care across entire care continuum.   GENERAL SUMMARY The WBSM Data Management team is responsible for the entire lifecycle of all data for the organization. Some of the primary duties of the team include: - Establishing automated processes to manage all data ingress and egress between payors, vendors, and applications. - Structuring and normalizing datasets within internal databases and file systems. - Querying and analyzing data to distribute insights to internal lines of business and external partners. - Designing and building sustainable and scalable dashboards, interfaces, and reports to inform on progress and gaps in operations. - Maintaining data integrity and quality across all systems and applications enabling our clinical and service teams to provide excellent care to every single patient. - Identifying opportunities to support, enable, and add efficiencies to the business’s operations. Wellbe is looking for a software engineer to join the Data Management team, primarily focusing on data transmission and integrations between enterprise applications. As our business is working in tandem with multiple health plans and vendors, there is a need to consume and distribute data to an ever-growing list of stakeholders. This role will be tasked with the design, development, and maintenance of software responsible for administering automated ELT and custom data driven solutions. This is a full stack position primarily focusing on the backend but requiring the ability to implement user interfaces where applicable.
