Job Summary

WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

 

OBJECTIVES

The Health Information Management (HIM) and Education Specialist is responsible for providing Release of Information (ROI), indexing, record recovery (“paper chase”), EpicCare Link monitoring, dismissal letters, and vaccine registry services.  This position may also work collaboratively with the New Market Launch team to assist in training and implementing health information management services in new WellBe markets.        

Job Description

GENERAL SUMMARY

  • Develops relationships with Emergency Department and Care Management personnel in the hospital and Skilled Nursing Facility settings.
  • Uploads and distributes daily incoming faxes as needed.
  • Distributes records and diagnostic results in a timely manner to appropriate practitioners.
  • Uses the AaNeel task function to communicate, coordinate, and manage medical record requests.
  • Monitors the daily census at assigned hospital/skilled nursing facility and requests/indexes records on an as-needed basis.
  • Completes “pull lists” (medical record audits, HEDIS, RAF, RADV).
  • Processes record requests related to: (1) legals, (2) government, (3) patients, (4) continuity of care, and (5) internal peer review/quality purposes; collaborates with the Compliance Officer and Executive Director, Enterprise Risk Management on Legals, government, and peer review requests.
  • Maintains state immunization databases for patient vaccinations.
  • Protects the security of medical records to ensure compliance is maintained.
  • Conducts market research of hospital data to analyze market trends.
  • Obtains access to medical records in the facility setting and assists with medical records transfers to WellBe practitioners for review.
  • Scans paper documents (when necessary) and indexes into the appropriate patient chart.
  • May serve as the market liaison between the Health Information Management and New Market Launch departments; as such attends periodic New Market Launch meetings.
  • Educates licensed and non-licensed practitioners and administrative staff on the process and policies for medical records.
  • Other duties as determined.

QUALIFICATIONS

Educational/Experience Requirements

  • High school graduate or equivalent
  • 6+ months of medical records experience
  • Basic understanding of records management
  • Knowledge of HIPAA and state release of information rules
  • Windows-based computer skills
