Job Description

Community Social Worker (MSW) Responsibilities:

 

  • Conduct psychosocial screenings with patients in person or via telehealth to identify social determinants of health and support needs.

  • Coordinate access to essential community resources and services based on patient needs.

  • Collaborate with Behavioral Health Specialists and the care team to support patient-centered care plans.

  • Participate in interdisciplinary team meetings to align on patient progress and care goals.

  • Document key interactions and care planning activities.

  • Maintain current written progress notes and other documentation on the member Medical Record including signed and dated documentation for all service in compliance with WellBe documentation policy.

  • Effectively collaborates with the family, primary caregivers, and appropriate community agencies to meet the patients’ care goals.

  • Collaborate and align with the community team on quality and performance measures and improving patient outcomes. 

  • Other tasks needed to accomplish team’s objectives/goals

 

Job Requirements

 

 

Community Social Worker (MSW) Qualifications:

 

  • Master’s degree in social work

  • Minimum of 2 years post-Masters clinical social work experience within the behavioral health field.

  • Have a valid driver’s license, automobile insurance, have access to an automobile; and proof of automobile insurance required.

 

Community Social Worker (MSW) Skills We Value:

  • Competency in working with older adults and knowledge of community services.

  • Familiar with EHR medical documentation 

  • Strong computer skills, including programs within the Microsoft program

  • Strong verbal and written communication skills

  • Bilingual in English/Spanish is a plus

  • Ability to work effectively in a team environment.

  • Knowledge of social work principles and practices, including case management and advocacy work.

  • Ability to leverage internal and external resources as part of a patient’s care plan 

 

                   

Supervisory Responsibility: This position will not have supervisory responsibility.

 

Travel requirements: Travel may be required up to 50% locally to deliver patient care. 

 

Work Conditions: Ability to lift 20lbs.  Ability to stand for extended periods.  Ability to drive to patient locations (i.e., home, hospital, SNF, etc).  Fine motor skills/Visual acuity.
