Job Summary

WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

 

GENERAL SUMMARY

We are looking for an ambitious Clinical Pharmacist to join our growing Medical Group team. As a Clinical Pharmacist, you will be responsible for ensuring the right and appropriate medication prescriptions for our patients. In addition, you will be responsible for different functions through collaborative drug therapy management and for providing outstanding patient care.

Job Description

SKILLS & COMPETENCIES

  • Participate in the development and implementation of clinical and operational pharmacy policies and procedures.
  • Assesses patient’s medication therapy regimen for appropriateness, effectiveness, tolerability, safety, and potential for adverse events.
  • Demonstrates proficiency in medication and disease management, as well as a thorough understanding of the medications, clinical conditions, and current treatment guidelines for the conditions in their assigned clinical areas.
  • Serve as medication therapy resource for activities such as drug information, training of staff, transitions of care and Medication Therapy Services.
  • Make clinically appropriate changes to regimen which will maximize effectiveness and patient satisfaction, while working to minimize potential for both acute and chronic complications Provides in-services, in-depth drug information, and other educational and informational materials to members of the medical, nursing, and other healthcare professionals.
  • Collaborate with healthcare professionals to ensure optimal patient care.
  • Ensure continuity of care and access with all medications.
  • Identify polypharmacy patients and strive to eliminate duplicate or unnecessary medications to reduce untoward interactions and reduce patient out of pocket costs while ensuring high quality use of medications.

Job Requirements

QUALIFICATIONS

  • Active pharmacist license (in good standing).
  • ASHP accredited PGY-1 Pharmacy Residency required
  • Strong knowledge of pharmacy laws, regulations, and best practices
  • Must be able to work independently as well as collaboratively with medical professionals
  • Strong organizational skills
  • Outstanding communication and interpersonal skills

PERFORMANCE INDICATORS

  • Polypharmacy Evaluation and reductions in utilization where possible.
  • Identify patients who may hit donut hole and offer alternatives to show them how to save money
  • Identify and work with non-adherent patients stressing importance of their targeted medications to improve health
  • Educate APC staff with lectures to provide a more in-depth knowledge of medications
  • Increase in the use generic substitution where appropriate
  • Oversight of drug interactions and its implications
  • Oversight of the overall Pharmacy spend within the Organization 

Supervisory Responsibility: No supervisory responsibilities.

 

Travel requirements: Travel may be required up to 15% locally or nationally.
