Job Summary

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

Job Description

The Implementation and Execution lead will be responsible for assembling and leading a cross-functional team  of clinicians and non-clinicians to stand up and launch new WellBe markets, implement new health plan contracts and new capabilities to standardize, mature and further enable our care model so our patients can have more good days.

 

The Director of Implementation & Execution is accountable for leading a interdisciplinary, cross-functional team in the hiring, training, implementing existing protocols and processes, necessary to enter new markets (geographies), contracts and/or new capabilities.

This leader will be responsible for developing and leading strategies to implement new capabilities across WellBe’s clinical service areas.  They will achieve desired results against key metrics (clinical, operational, and financial) and ensure stabilization and seamless transition of new market operations and/or capabilities into the base-business.  Additionally, this role will be accountable for ensuring standardization within and across markets consistent with WellBe’s standard operating model nation-wide.

This position will require travel depending on the complexity and size of implementation. For new capabilities, this leader will be responsible for scoping, planning and managing cross-functional teams to build and deploy new capabilities.  This position will need to ensure that new capabilities are adopted by core-operations.

This leader’s primary responsibility is to provide oversight to an interdisciplinary team of physicians, advanced level practitioners and other key operational delivery roles required to stand up new markets and serve the markets patient population. This role will report directly to the VP of Enterprise Priority Office  and must work collaboratively with key leaders across operations, clinical, technology, and analytics teams to deliver against their intended scope.

 

The Director of Implementation and Execution’s objectives will include:

  1. Create a standardized approach [playbook] to the implementation & launch of markets and capabilities in order to deliver consistent value delivery and patient experience
  2. Deliver accelerated implementation timelines
  3. Support new market and capability stabilization post go-live
  4. Identify and implement continuous improvement opportunities to streamline and gain efficiencies in WellBe’s execution & implementation processes

 SKILLS & COMPETENCIES

The Director of Implementation & Execution will enable the enterprise to assemble and direct the right interdisciplinary team to effectively and efficiently stand-up new markets, implement health plan contracts and introduce new capabilities without impacting day-to-day operations.   Outside of implementing new markets and health plans, this role will partner closely with key leaders to launch and drive results from new service lines and capabilities. Weekly activities may vary depending on the business needs.

  • Upholds the WellBe Care model in each market when training and shadowing new hires for the new market to ensure standardization and clinical competency

  • Ensures timely ramp-up and training of key stakeholders and team members based on the nature of the implementation
  • Implements standardized clinical and operational best practices in new and existing markets
  • Establishes standard operating cadence for new markets and capabilities
  • Works collaboratively across the business to define, optimize, and evolve standard operating procedures and business process standardization across the enterprise
  • Strong verbal and written communication and interpersonal skills, including demonstrated ability to communicate effectively and tactfully to internal and external stakeholders
  • Possess highly effective time management and organizational skills to manage highly skilled team to achieve multiple objectives and meet deadlines
  • Work and collaborate effectively within a team environment

  • Serve in a leadership role, providing subject matter expertise, guidance, and direction to cross-functional teams and peers

Job Requirements

Educational Requirements:

  • Bachelor’s Degree with in business or related field 

Required Skills and Abilities:

  • 7+ years’ work experience in healthcare operations (physician practice and/or home health operations preferred)
  • 10+ years successful supervisory or management experience, including delivering results by leading via influence vs. direct line-management (Required)
  • Full risk knowledge or value-based medicine experience
  • Demonstrated proficiency with Microsoft Office; Excel (e.g. graphs, pivot tables, etc.), Word, PowerPoint, G Suite,
  • Demonstrated experience and comfort level with presentation skills
  • Demonstrated understanding of the theories of constraint and six-sigma processes and program management
  • Ability to work with stakeholders within and outside organization to resolve areas of concern
  • Must be able to analyze data, determine trends, formulate action plans and execute plans

Supervisory Responsibility: Position has supervisory responsibilities.

 

Travel requirements: Travel may be required up to 10% locally or nationally

 

Work Conditions: Ability to lift up to 20lbs.  Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs. 

  • Ability to stand for extended periods
  • Ability to drive to patient locations (ie. home, hospital, SNF, etc)
  • Fine motor skills
  • Visual acuity
