Job Summary

WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care provider to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

Job Description

GENERAL SUMMARY

Our Patient Care Medic (PCM) are the energetic, curious explorer, part of a team that provides compassionate care to the frailest elderly who suffer from chronic and acute illnesses or injuries living in their homes.  Our PCM is responsible for developing a rapport, and delivering exceptional in-home services, helping coordinate care with the rest of the medical team, and contributing complete and accurate data in patient's records. The PCM is trained and equipped with specialized tools and supplies to handle routine and emergency situations effectively. Empathy and compassion towards the patients are integral qualities expected from the candidate. A customer-centric approach and strong communication skills, both with patients and team members, are necessary for success in this role. The successful candidate will love working with seniors and a collaborative interdisciplinary team. SKILLS & COMPETENCIES

  • In-home assessments, screenings, and ongoing home visits under Provider direction
  • Conducting and documenting medical history, medication inventory, and physical exam
  • Conducting and documenting home safety inspections
  • Administering medication (incl. injections/vaccinations)
  • Appropriate wound care
  • Perform respiratory evaluation and provide nebulizer treatments
  • Point of care testing and phlebotomy for commercial lab testing
  • Starting IVs and administering IV medications (Will train if not currently certified.)
  • Foley catheter and PEG care
  • Coordination with Medical Control and patient care team (incl. physicians, PCAs, specialists, etc.) in all patient care to deliver treatment under specified and approved protocols
  • Expedient coordination with EMS and ER personnel as necessary to arrange emergency care
  • Conducting appropriate in-home diagnostics incl. ECG, PulseOx, blood glucose, spirometry, etc.
  • Operating and maintaining company equipment per applicable safety rules and manage patient medical records as appropriate
  • Provide positive customer service and treat all patients and staff with respect
  • Other tasks needed to accomplish team’s objectives/goals

Job Requirements

QUALIFICATIONS

 

Educational/ Experience Requirements:

  • Licensed Practical Nurse Required (Registered Nurse preferred)
  • 3+ years of experience working for a healthcare practice or hospital 
  • Military Medic or Urgent Care experience preferred
  • Prior experience delivering in-home patient care, community paramedicine / mobile integrated health experience, or urgent care experience working in EDs, or primary care clinics
  • Prior experience working in an interdisciplinary care planning team focused on providing care for frail, complex patients

License, Certification, Registration

  • LPN License Required (RN Preferred)

Required Skills and Abilities:

  • Current BLS card for healthcare providers or willing to obtain 
  • EMR documentation experience preferred
  • High level of professionalism 
  • Ability to work independently, problem solve efficiently and execute required tasks with strong attention to detail
  • Strong communication and interpersonal relationship development skills
  • Strong verbal, written, presentation, and interpersonal communication skills             

Supervisory Responsibility: N/A

 

Travel requirements:Travel is required locally. The role is a field-based role visiting patient homes.

 

Work Conditions: Ability to lift up to 20lbs.  Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100bs.  Ability to stand for extended periods.  Ability to drive to patient locations (ie. home, hospital, SNF, etc).  Fine motor skills/Visual acuity
