The Clinical SW Supervisor will work to ensure market social workers are educated, motivated, and aligned with the WellBe model of care. The Clnical Supervisor will ensure all social workers in market territories are properly engaged and maintain appropriate communication with those necessary.   Our social workers are part of a team that provides compassionate care to the frailest elderly who have chronic and acute illnesses or injuries living in their homes. Our LCSW delivers primary, urgent and acute care and counseling to a wide variety of patients with social and behavioral problems. Additionally, will be responsible on how to use along with their team; the daily operating systems/tools, familiar and adhere to company policies and procedures, address questions/complaints (team and members), and drive change management. Also assist in hiring new employees while monitoring their team's success on an ongoing basis. Supervisors will also need to assist with guiding their team as it relates to quality assurance, keeping tallies of scheduled visits and pulling call lists.

The ideal candidate will have a solid knowledge/understanding of healthcare data sets, including medical claims, HCC, HEDIS, and be detail oriented.  This role reports to the Chief Executive Office and works closely with operational, IT and clinical leaders. Responsibilities include collaborating with finance, operational, IT and clinical leaders to design analyses to answer key questions about our care model, complete in-depth analyses leveraging medical claims, EMR inputs, patient attributions, and healthcare operations data sets.   Identify key insights and communicate to various stakeholders through the WellBe organization.

Accurately noting all patient information according to confidentiality standards and HIPPA requirements.  Routes calls to the appropriate departments as needed.

This leader will help provide guidance, focus and execution to the strategic direction of the Outreach Center to include but not limited to inbound, outbound, quality and training department development and direction. Work as a cross-functional leader to build transparent, impactful, and collaborative member related services within WellBe.  Achieve results, drive initiatives to improve the efficiency and effectiveness of processes and procedures, seek out best practices to support scalable growth with a focus on people, processes, and technology.  Establish a culture that supports best in class service to our membership and is aligned to the WellBe core values. You will have the opportunity to shape and execute the WellBe outreach team, strategy, and leadership reporting to you.  You must have proven  management experience and a passion for patient satisfaction.
