Job Summary

WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

 

GENERAL SUMMARY

The role would bear the responsibility and oversight of the behavioral health component of the patient care. Accountable for the overall delivery, safety, quality, and consistency of patient care rendered by the LCSW and other behavioral health employees.

  • Monitors the performance and outcomes of the regional LCSW
  • Responsible for staff training activities that promote the professional development of team members.
  • Monitors quality performance measures through regular chart audits and reports. Review of the patient care plan and clinical execution of on-going care
  • Accountable to educate staff on the WellBe Care Model and how their role makes a difference in the overall patient care.
  • Manages the development and implementation of social and behavioral health
  • Advises leadership on improvement opportunities regarding social and behavioral health programs.
  • Provides support to LCSWs on complex case management for patient and families requiring a higher level of services.
  • Constant review of social worker intervention and assessment of social support in the transitions of care, end of life discussions, addressing HEDIS gaps (med adherence) and palliative care interventions.
  • Manages hiring of staff for all behavioral health care
  • Accountable for clinical implementations, ongoing monitoring of success of the initiative
  • Creates and manages the national
  • Collaboration with care team leaders on the care of the patients
  • Provide support/share knowledge with the team with high intensity psychotropic medications being prescribed/managed by WellBe

Job Requirements

QUALIFICATIONS

  • Experience with behavioral health and social work, preferably working directly with patients and their family members
  • 7+ Years’ Clinical supervision experience
  • Current active social worker licensure, clinical or independent licensure (preferred)
  • County or state experience preferred
  • Experience in program development and implementation
  • Interpersonal, critical thinking, and organizational skills
  • Understanding of a full-risk or value-based care
  • Understanding of Risk Assessment and Quality measures

Educational Requirements:

Psychologist or independently licensed social worker

Supervisory Responsibility: No supervisory responsibilities.

 

Travel requirements: Travel may be required up to 25% locally or nationally

 

Work Conditions: Ability to lift up to 50lbs.  Moving lifting or transferring of equipment may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs. 

  • Ability to stand for extended periods.
  • Ability to drive to patient locations (i.e. home, hospital, SNF, etc.)
  • Fine motor skills
  • Visual acuity
