Job Summary

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

 

GENERAL SUMMARY

The Regional Vice President New Market Launch Team will be a key member of the Market Launch Team, which will be responsible for standing up new markets. The role will be accountable to train, hire, and implement existing protocols and processes, and precept staff until the market is functioning autonomously. Market Launch Team members will be responsible for key metrics (clinical, operational, and financial) until the new market leadership team is established. This position requires extensive travel (i.e. 3 continuous weeks per stay at designated location).

Job Description

SKILLS & COMPETENCIES

The Regional Vice President New Market Launch Team will serve as an integral part of the implementation efforts, support mature markets, , areas of growth, and new service line launches' to all the launch team JDs.. Weekly activities may vary depending on the business needs.

 

GENERAL

  • Upholds the WellBe Care model in each market when training and shadowing new hires for the new market to ensure standardization and clinical competency
  • Practices the WellBe mission: To help our patients lead healthier, meaningful lives by delivering the most Complete Care ©.
  • Work with new hires to provide a deep understanding of the WellBe culture and values

RECRUITMENT & HIRING

  • Works with Recruitment team to help source potential candidates for open positions in new markets
  • Key resource in interview potential candidates and determine hiring permanent market field team
  • Works with preboarding as needed to ensure that new hire is prepared for the first day of New Employee Orientation.

ONBOARDING & IN-MARKET TRAINING

  • Works closely with the Regional Vice President and Training and Development Team to onboard APCs
  • Conducts in-market provider training, including classroom training, and ride-alongs for any 30/60/90 clinical training plans
  • Completes provider competency checks post market implementation
  • Responsible for continued training staff on:
    • Frequently utilized clinical technology systems
    • Performing effective initial comprehensive assessments on newly engaged members (including use of all WellBe tools, e.g., QuantaFlo, CWV, all applicable screenings, etc.)
    • Conducting effective Transitions of Care aimed to ensure seamless transition into the home environment and decrease risk of readmissions
    • Advanced directives, goals of care, and end of life conversation to patients/families, and caregivers
    • Processes and policies with the mobile integrated health paramedic partners when applicable
    • Handling urgent and critical situations to provide stabilization, triage, and hospital at home care planning where appropriate
    • Appropriately ordering, interpreting, and analyzing diagnostic tests and determining the need for follow-up appointments and further treatment options
    • Appropriately prescribing and managing medications and treatments, including but not limited to psychopharmacological agents
    • Conducting root causes analyses of suboptimal patient and community resource(s) outcomes to develop care plans to achieve optimal patient outcomes
    • Developing medical, psychosocial, emotional, spiritual care plans and urgent treatments while maintaining effective time management and care strategies
    • Delivering value-based healthcare, identifying HCCs, and filling HEDIS gaps
    • Use of evidence-based educational tools like UptoDate and DoctusApp
    • Collaborate with training team members to operationalize additional processes

ONGOING NEW MARKET SUPPORT

  • During ramp up of a new market, this role will provide
    • Clinical care to a small patient care panel
    • Activities to accelerate engagement activities
  • Implements standardized clinical best practices in each new market
  • Serves as a preceptor and resource for new providers, helping them learn the “WellBe”
  • way
  • In collaboration with the Community Medical Director, temporarily provides the oversight of the day-to-day clinical care provided throughout the community
  • Ensures clinical excellence, extraordinary patient and provider experience, quality outcomes, and utilization goals are met, until market leadership can function autonomously
  • Coaches, mentors and counsels APCs and SWs to meet performance levels, and foster growth of “Care Community”
  • Establishes relationships/works with Health Plan agents, specialty networks, hospitals, nursing homes, etc.
  • In collaboration with the newly hired staff, establishes weekly market meetings: IDTs, staff meetings, clinical rounds, etc.

EXISTING MARKET SUPPORT

  • Supports any organizational process improvement plans to mature markets
  • Supports and participates in enterprise-prioritized projects and programs as needed

Job Requirements

QUALIFICATIONS

Educational Requirements:

  • For a Nurse Practitioner: Graduate of an approved Nurse Practitioner program (Required Must have or be eligible for appropriate APP licensure).
  • For Physician Assistant: Graduate of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) (Required Must have or be eligible for appropriate PA certification).
  • Master’s degree in healthcare discipline required if you are a physician assistant.
  • CPR certification (Required).

Required Skills and Abilities:

  • 10+ years successful supervisory or management experience (Required).
  • 3+ years' experience in a health care organization.
  • Full risk knowledge or value-based medicine experience.
  • Previous experience in home care, hospice, palliative care, population health, or geriatrics strongly preferred, other complex patient health care experience also beneficial.
  • Demonstrated proficiency with Microsoft Office; Excel (e.g. graphs, pivot tables, etc.), Word, PowerPoint, G Suite,
  • Demonstrated experience and comfort level with presentation skills including proficiency with
  • Demonstrated understanding of the theories of constraint and six-sigma processes, program management and
  • Ability to work with stakeholders within and outside organization to resolve areas of concern.

Supervisory Responsibility: Directly oversees staff within an assigned geographical area.

 

Travel requirements: Travel may be required up to 60% locally or nationally
